Our minimum age requirement is 12 years old, and patients must have all permanent teeth present. For patients under the age of 18, we need a parent/guardian signature on the Medical/Dental History Form provided during the evaluation step.

Yes, you may be eligible for coverage from your insurance company. Use the instant eligibility tool on our  to find out exactly how much your provider will cover. In most cases, SmileDirectClub will coordinate payment directly with the insurance company, so you only pay the balance of what you owe after insurance coverage is applied to your total treatment cost. There are some cases where your insurance company may require that you pay the total balance of your aligner treatment before applying for reimbursement through them. If so, we will provide the forms you need to file your reimbursement claim with your insurance provider.
